Three hospitals that had beenin operation for more than five years agreed to participate in this study. An interview survey wasadministered to two outpatient nurses selected from each hospital by its executives (a total of six nurses).The interview was recorded using an integrated circuit recorder, and the recorded interview was thentranscribed verbatim after obtaining the consent of each nurse. The obtained data were analyzed usingqualitative induction. Nurses were all female, with an average age of 47.3±5.0 years. Regarding thecurrent situation of and problems with seamless care for elderly patients with dementia home recuperation,six categories were extracted: (i) the practice of seamless care through multi-occupational descriptioncooperation in the community, (ii) the role of an outpatient nurse according to the cognitive capacity ofa patient, (iii) home recuperation, (iv) care after recognizing dementia, (v) improvement in dementia careskills as a professional, and (vi) the circumstances of a patient?s family. To continue home recuperation,seamless care for elderly patients with dementia through medical treatment and support for patients?everyday life is required.Keywords : elderly patients with dementia, Outpatient nurses, Seamless care, Community-based integratedcare systemsⅠ.IntroductionIn Japan, the number of elderly persons withdementia has increased, and it has been estimatedto be 3.45 million in 2015, and to reach 4.10million in 2020 and 4.70 million in 2025 6) . Thistendency is also exhibited in other countries. Thus,the number of elderly persons with dementia hasincreased worldwide. According to the WorldAlzheimer Report 1) ,“Today, over 46 millionpeople live with dementia worldwide, more thanthe population of pain. This number is estimatedto increase to 131.5 million by 2050.”Social resources such as long-term careinsurance have been developed in Japan.
